Pomerene Hospital and the Holmes County Health Department (Partners for a Healthier Holmes County) will be conducting a Community Health Assessment this February. Over the next two months, nearly 1,200 adults living in Holmes County will be receiving a health survey by mail. Any adult in Holmes County may receive this survey. The surveys will arrive in a large white envelope containing a survey booklet, a pre-addressed, pre-stamped envelope to return the completed survey, and a $2.00 bill for your participation.
The anonymous survey asks participants to answer questions about general health, risk and protective health factors, and access to health care. For those that receive and complete the survey, all answers will be anonymous and confidential. Only group results will be analyzed. The results of these surveys are very important in helping local agencies identify key health problems and needs of Holmes County residents. Please watch for the release of the final health assessment report in the fall of 2017.
Pomerene Hospital and the Holmes County Health Department encourage those who receive a survey to participate. It is important that every adult in Holmes County do his or her best to help local organizations identify the health priorities that are impacting the community. This assessment will help local health organizations plan health programs based on the needs of the community. For those receiving and completing a survey, please mail responses shortly after receiving the survey in the mail so data can be compiled in a timely fashion.
Pomerene Hospital and the Holmes County Health Department thank all those in advance for taking the time to help with this very important project.
