Patient Information

Protecting your privacy and the confidentiality of your health information is a top priority at Pomerene Hospital.

At Pomerene Hospital, like every other hospital throughout the country, we now provide a Notice of Privacy Practices that explains our policies for ensuring your confidentiality and privacy in the use and disclosure of all patient health information.

For example, as a patient admitted to the hospital, we now ask if you would like to be included in the our hospital directory. If given your permission, we are then able to record the following information within this directory:

     Your name;
     Room number (in limited circumstances);
     Condition (in general terms); and
     Religious affiliation

You can refuse inclusion in the directory and/or request only portions of this information be listed. The hospital can then give out patient-permitted directory information only to those who ask for you by name. Religious affiliation is available only to clergy.

Therefore, if a visitor comes to the hospital and asks for you by name, we will confirm you are in the hospital and provide the information you have given us permission to share. However, if you indicate a desire to not be listed in the directory, we will not give any information about you to anyone who asks, even to say yes or no as to whether you are a patient.

Additionally, your health information is limited only to those who are involved in your care and those monitoring quality of care and safety issues. We go to great lengths to discuss with you and/or your family the circumstances of your illness or injury with immense discretion and privacy. And when your health information must be shared with outside sources, such as your insurance company, numerous safeguards are in place to ensure the secure transmission of that information.
