Pomerene's Diabetes Program Sees Continues Success in Its Third Year

(Millersburg, OH)- Now in its third year, Pomerene Hospital’s Comprehensive Diabetes Management Program continues to grow.

Pomerene Hospital’s Comprehensive Diabetes Management program, which is accredited by the American Association of Diabetes Educators, allows residents in and around Holmes County increased access to critical diabetes education services.
Diabetes education is a collaborative process through which people with or at risk for diabetes gain the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions.
Pomerene’s program works in collaboration with the individual and their healthcare provider to optimize their skills and knowledge for better management of diabetes. The program consists of a Registered Nurse and Dietitian and offers a variety of services including in-patient education, out-patient education, individual training or training in group settings. To take part in the program, a physician referral is required.

“Our goal at Pomerene Hospital is to empower our diabetic clients with the self-management skills necessary to improve their health and well being.  We are here to guide, educate, and support our community toward better health,” said Kathy Snyder, RN, and Diabetic Education Coordinator at Pomerene Hospital.

In the last year, Pomerene has had 33 patients complete the program.

Most recently, Pomerene Hospital submitted an Annual Status report, as required, per the Compliance Enforcement Policy of AADE, and was found to remain in compliance with the national standards for AADE DSME/T.

 “It is very exciting to be a part of people’s success in managing their diabetes,” reports Snyder.

According to the 2014 National Diabetes Statistics Report, 29.1 million people in the United States have diabetes. Twenty-one million of those are diagnosed and 8.1 million are undiagnosed.
